Well-Integrated Screening and Evaluation for Women Across the Nation
Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN)Program, Heart of the Family/Corazón de la Familia, receives funding from the Centers for Disease Control and Prevention. The priority of the WISEWOMAN Program is risk reduction with a focus on hypertension control, which aligns with and supports Million Hearts.
WISEWOMAN is a sister program to Every Woman Counts, Department of Health Care Services' Cancer Detection and Treatment Branch. Working together, the programs offer women access to services in one location. WISEMAN operates at the local level providing low-income, and under-insured women, 40 to 64 years of age, with chronic disease risk factor screening, health coaching, and community referral services in an effort to prevent cardiovascular disease.
The overarching goal of WISEWOMAN is to provide women with the knowledge, skills, and opportunity to improve diet, physical activity, and other behaviors in order to create lasting, healthy life habits to prevent, delay, or control cardiovascular and other chronic diseases.
Program sites currently participating in Heart of the Family during 2013-2017 are:
- Logan Heights Family Health Center of San Diego, San Diego County
- Northeast Valley Health Corporation, Los Angeles County
- Vista Community Clinic - Vale Terrace, San Diego County
- As funding permits, expansion to other California locations is planned.
