Skilled Nursing Facilities 
COVID-19 Daily Reporting

The California Department of Public Health (CDPH) has created an online survey for skilled nursing facilities (SNFs) to report COVID-19 information to ensure that California has the information necessary to respond to the COVID-19 outbreak and to provide resources and support to providers.  CDPH will report the SNF facility data to the Centers for Disease Control and Prevention (CDC) on behalf of providers.  Failure to report in accordance with 42 CFR 483.80(g) can result in an enforcement action (F884 and F885).

SNF COVID-19 reports to CDPH are due by 12:00 P.M. Pacific Time, daily. 

For reports before June 22, 2020, 12:00 P.M, please use the SNF COVID-19 Survey 2.0.

For reports starting June 22, 2020, 12:00 P.M., please use the SNF COVID-19 Survey Hub via the button below:

​Note: The SNF COVID-19 Survey Hub will not be live until June 22, 2020, 12:00 P.M. The SNF COVID-19 Survey 2.0 link will no longer work beginning June 29, 2020. Refer to  AFL 20-43.3 for full details.
