× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

​​Laboratory Personnel Licensing  

First Time Applicants:

California state law requires that licensed clinical laboratories in California employ testing personnel who are licensed by the State.  The new personnel license application process requires the steps below.

Request access to the Laboratory Field Services application. 
 

LogIn 2. Log into your account

Log in to your account, create an application, sign the attestation page, upload documents, continue an incomplete application, or check the status of an application.


Laboratory Field Services (LFS) fees are non-refundable and are subject to change with each fiscal year. Refer to the LFS fee schedule (PDF) for a full list of fees associated with clinical laboratory facility registration and licensing.

Applying After Abandonment:

If you do not submit all of the required documents for your application, it is possible that your application will be abandoned. In this case you will need to reapply for your license/certification. In order to apply, you will need to follow the steps listed below:

  1.  Log into your account 

  2.  Under the "Need a License/Certificate? Apply Here" section, click the "Apply Now" button.

  3.  Complete the application. 

 

Renewal Applicants:

Attention Renewal Applicants:

NEW Download and Print Personnel License

Effective June 18, 2019  you will be able to download and print your active license. Personnel Licensing will no longer mail your license or certificate. This means you will no longer need to wait for your license through the mail and you can easily print your license or certificate as many times as needed, provided it is active.

Online Renewal Applications

On November 14, 2017, Laboratory Field Services (LFS) launched an update to PERL, the online application system for personnel licenses and certificates, allowing licensees and certificate holders to submit renewal applications online. For additional information and guidance please visit the LFS personnel renewal web page.

  • LFS will accept paper applications that are postmarked by November 30, 2017; any applications mailed after November 30, 2017 will not be processed, and applicants will be directed to resubmit their renewal applications online.
  • All renewal applications submitted via the online system must include applicants' continuing education certificates and/or unofficial transcripts.
