× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

Create an Account and Log In (User Manual)

To create an account, go to: Create An Account (https://mylicense.cdph.ca.gov/prweb/PRWebLDAP1)
You can also go to our website (cdph.ca.gov/LabLicensing) and click Log In. Then, click Create an Account.

step 1Complete the required fields. Know the password requirements to avoid error. Click Submit.

step 2Take note of your username. Click Close to proceed to the log in page.

step 3Log in using your username and password. Click Log in.

step 4Click the Laboratory Facilities icon.

step 5Read and accept the Privacy Disclaimer by checking the box. Click Proceed.

Step 6 If you do not have another account with CDPH, the system will ask you if you are an owner of the business you are about to manage online or not. Simply answer Yes or No and complete the required fields. If you have an account with CDPH, proceed to Record Association steps 7-12.

  1. Not an owner: Select No and click Submit.
  2. Review your Key Demographics information and click Submit. Proceed to step 11.
  3. Owner: Select Yes and click Submit.
  4. Enter your SSN or ITIN and click Submit. Proceed to step 11.

Record Association

Step 7 If you have another account with CDPH, the system will recognize your email address and ask you to associate your account to your existing account. Review the record if correct, select the email address. Select This is me and click Proceed.

Step 8 Answer the Citizen Association security question to merge your account with the record selected by entering your SSN and Date of Birth. Click Search.

Step 9 If the merge was successful, you will see a confirmation message. Click Proceed.

Step 10And finally, confirm your Key Demographics information and click Submit.

Step 11 On your welcome screen, you can search and link the business you need to manage or start a new license application.
