× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

In order to increase processing efficiency and decrease wait times, CAIR2 has created an online form that allows patients/parents to determine first whether a matching patient record exists in CAIR2 and if a match is found, to complete the full record request online. In order for the record to be released, the requestor will also need to upload a file containing a scanned driver’s license or other official picture ID and any other relevant documentation authorizing release of the requested record. Please have these files ready to upload as the Record Release form must be completed in a single session. Please note that finding a matched patient in the initial CAIR2 quick search does not guarantee that the actual requested patient record will be found.

Please note that this form is for accessing CAIR2 immunization records only. Contact the HealthyFutures registry for patient records in the greater San Joaquin Valley.
