NCAA Athletes and the California Newborn Screening Results for Sickle Cell Trait
The California Newborn Screening (NBS) Program has been screening all babies in California for sickle cell disease and sickle cell trait since February 27, 1990. These results will satisfy the NCAA requirement for college student athletes to have documentation of their sickle cell status.
*** It can take up to 30 days to process your request. If you need your results in less time, we recommend having a sickle cell trait test run by your physician.
*** Results are only available for California births on or after 2/27/1990.
To request results, please follow these steps:
Enter data directly in to the online form
CDPH 4400 (PDF). MAC users will need to print form and enter data by hand.
Print and Sign form – please read below regarding who must sign form
a. Parents cannot request results for offspring 18 years or older.
b. If the student is 18 or older, the student must sign the form.
c. If the student is 17 or younger, the parent/guardian must sign.
d. Include the email address of the signer for reporting of results.
e. Results cannot be processed if there is an incorrect signature (due to Federal & State HIPAA regulations).

Q: Will you send a copy of my results to my coach?
A: Yes, simply list the email address for your coach in the “Release Records to” section of the form.
Q: It’s been a month! Please! Where are my results?!
A: Email us at
NCAANBSresults@cdph.ca.gov. This mailbox is monitored regularly. Please remember, due to the number of requests received, it can take us up to 30 days to process each request. We are unable to expedite results due to special circumstances. If you need your results sooner, please contact your physician for sickle cell trait testing.
Q: Do I have to fill this out next year, too?
A: No, you do not have to resubmit this form every year if you have already received your results. Sickle cell trait results, like blood type, do not change.
