Find and Choose a Nursing Home
Health Facilities Consumer Information System (HFCIS) website provides health care consumers and the public with information about licensed long-term care facilities and hospitals throughout California. The website’s content is made available by the California Department of Public Health, Center for Health Care Quality, Licensing and Certification Program (L&C). Information provided includes facility ownership, certification status (acceptance of Medicare and/or Medi-Cal) and performance history (complaints, facility self-reported incidents, state enforcement actions, and deficiencies identified by L&C staff). A nursing home can be located by entering a zip code, city, or county.
In addition to numerous links to other governmental, advocate and provider organizations, HFCIS provides consumers and the public with an
online complaint form to file a complaint regarding a specific facility. Upon submission, the complaint is forwarded to the appropriate Licensing and Certification District Office for investigation.
