āāFacility Alert: Man Poses as a Certified Radiologic Technologist
A
man identified as Keith Harold Moultry, aka: Kenneth H. Moultry, d.o.b.
12/29/1972, has been posing as a certified Radiologic Technologist
using false identification and credentials to gain employment in
California medical facilities. Moultry is African American,
approximately 5'11, 175 lbs., and reportedly has the word "Brooklyn"
tattooed on the left side of his neck.
The California Department
of Public Health, Radiologic Health Branch (RHB) has been tracking
Moultry's actions and providing aid in identification to facilities who
have been victimized.
Moultry has been gaining employment in San
Bernardino and Los Angeles County hospitals, clinics and medical offices
using false identification and credentials. He is adept at creating
identification and other documents that appear genuine and official.
Although his documents appear official, the details of the documents are
often inconsistent and contradictory and the explanations for these
inconsistencies are usually far-fetched and hard to confirm. Moultry
has an extensive record of arrests and has been convicted of multiple
felony offenses, including burglary, false imprisonment, identity theft,
grand theft, and sexual battery. He poses a risk to patient health and
safety while working at medical facilities under false pretenses.
RHB strongly recommends careful scrutiny of the name associated with the social security number and to obtain fingerprints to corroborate the identity of all new hires prior to allowing individuals to begin work. Prompt and consistent implementation of such precautionary measures will help ensure patient and employee safety.
