Vital Records Fees

Requests received without the appropriate fee will be returned to the sender. Make your check or money order payable to CDPH Vital Records. Checks must be drawn on a United States bank. Money orders must be drawn on a United States bank or issued by the United States Postal Service.

Please do not send cash by mail. CDPH cannot be held responsible for fees paid that are lost, misdirected, or undelivered.

VS 21 – Adjudication of Facts of Parentage VS 22 – Acknowledgement of Paternity/Parentage VS 23 – Court Order Name Change VS 85 – Delayed Registration Birth VS 108 – Court Order Delayed Registration of Birth	$26 amendment fee and includes one free copy of newly amended record. $29 per additional copy
VS 24 – Affidavit to Amend a Birth Record* VS 107 – Supplemental Name Report-Birth *The fee to change/correct the sex field or parent designation on child’s birth certificate is $26, regardless of when the request is submitted.	Submitting within one year from date of birth No fee to register the amendment and does not include free copy. $29 per copy Submitting after one year from date of birth $26 amendment fee and includes one free copy of newly amended record. $29 per additional copy
VS 24C – Affidavit to Amend a Marriage Record *The fee to change either spouses’ designation as bride, groom, or having neither box checked to reflect gender identity is $26, regardless of when the request is submitted. VS 44 – Court Report of Adoption	Born in CA or born outside of the US/Canada and adopted in CA No fee to register the amendment and includes one free copy of newly amended record. $29 per copy Born in CA and adopted out of state $20 to register the amendment and includes one free copy of newly amended record. $29 per additional copy
VS 105 – Court Order Delayed Registration of Fetal Death	$26 amendment fee and includes one free copy of newly amended record. $21 per additional copy
VS 109 – Court Order Delayed Registration of Death	$26 amendment fee and includes one free copy of newly amended record. $24 per additional copy
VS 122 – Court Order Delayed Registration of Marriage	$26 amendment fee and includes one free copy of newly amended record. $17 per additional copy
