Local Maternal, Child and Adolescent Health

Annual Reports (for Fiscal Year 2017-18)

All Local Health Jurisdictions (LHJs) are required to complete and submit an Annual Report for the following programs:

  • Maternal, Child, and Adolescent Health (MCAH) Program
  • Fetal Infant Mortality Review (FIMR) Program
  • Sudden Infant Death (SIDS) Program
