New Nurse Assistant, Home Health Aide and Hemodialysis Certificates
and Updated Applications and Forms



ā€‹Effective immediately, a redesigned certificate will be issued to certified nurse
assistants, home health aides, and certified hemodialysis technicians. These new
certificates are being phased in as existing certificates expire. Current certificate
holders will not be re-issued a new certificate until they become eligible for renewal.
The current design for all existing certificates remains valid until those certificates
expire.

Please see the sample of a nurse assistant certificate below. The sample certificate
below is not the actual size.

Each new certificate is designed with special pressure seal stock paper which includes
several security features. The California Department of Public Health (CDPH) is
encouraging all certificate holders to sign their certificates upon receipt and before
presenting the new certificate to potential employers. CDPH also recommends that
certificate holders DO NOT laminate their new certificates due to the security features.
Laminated cards make it difficult to view certain security features and therefore difficult
to verify the authenticity of the card. However, it is still the responsibility of each
facility to verify current certification status by using the online verification at
http://www.cdph.ca.gov or call the Registry at (916) 327-2445.
We have also updated many of our applications and forms and have made them
available online. All facilities and training programs are advised to obtain the latest
copies at the website listed above.
