× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

Please be wary of potential highly partisan political messaging while visiting federal government websites for information related to the federal government shutdown.

TO:
All Facilities

SUBJECT:
Centralized Applications Unit: Facility Name Change with Submission of a Change of Ownership (CHOW) Application

All Facilities Letter (AFL) Summary

This AFL is a reminder that a facility may not use a new facility name until the Centralized Applications Unit (CAU) completes all CHOW-related approvals.

State law requires a facility to submit a new licensing application for any CHOW. If a facility is requesting a name change when submitting the CHOW application, the facility may not use the new name until CAU has approved the CHOW application, Licensing and Certification has conducted the survey, and CAU has issued the license.

Only the current licensee of a facility can request a name change. The CHOW application is subject to a full application review and approval process. Health facility licensing and/or certification applications are available at:

For additional questions regarding the CHOW approval process or a name change request, please contact CAU by phone at (916) 552-8632 or by email at: CAU@cdph.ca.gov

Sincerely,
