How to Request Verification of Your California License:
I am applying for a Genetic Counselor license in
another state, and they are requiring verification of my California license;
how will I obtain this information?
If you need a verification letter stating that you have a Genetic Counselor license in good standing with the California Department of Public Health, Genetic Disease Screening Program, you will need to logon to your existing Genetic Disease Screening Program Portal account; and click on the download License Verification button.
If you are requesting license verification for a state that requires our state seal and/or their form to be completed by our department, you will need to complete the CDPH Form 4491 and provide any additional paperwork required by the state that is requesting verification.
