× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

Transfer of Skilled Nursing Facilities (SNFs) to the San Jose District Office (DO).



​All Facilities Letter (AFL) Summary

This AFL provides notice that the San Francisco DO is transferring the oversight responsibility of 20 SNFs in Santa Clara County to the San Jose DO.

​Effective January 15, 2017, the San Jose DO will be responsible for the oversight of the following facilities:

​2. Channing House ​12. Saratoga Pediatric Subacute
​3. Cupertino Healthcare & Wellness Center ​13. Saratoga Retirement Community Health Center
​4. Grant Cuesta Sub-Acute and Rehabilitation Center ​14. Sunny View Manor
​5. Health Care Ctr at the Forum at Rancho San Antonio ​15. Sunnyvale Post-Acute Center
​6. Idylwood Care Center ​16. The Terraces at Los Altos Health Facility
​7. Los Altos Sub-Acute and Rehabilitation Center ​17. Valley House Rehabilitation Center
​8. Manorcare Health Services (Sunnyvale) ​18. Vi at Palo Alto

Following the transfer, all complaint and entity reported incidents must be reported to the San Jose DO.
