Name Change for the Center for Health Care Quality, Licensing and Certification (L&C), Centralized Applications Unit (CAU)

All Facilities Letter (AFL) Summary

This AFL notifies health care facilities that the L&C, CAU name and associated email address has changed.

In an effort to improve efficiency, performance, and services to our providers, the Center for Health Care Quality, L&C is pleased to announce that CAU has expanded and reorganized to the new Centralized Applications Branch (CAB) effective August 14, 2018. The new branch consists of four sections:

Non-Long Term Care Section
Long-Term Care Section
Report of Change Section, and
Licensing-Certification and Policy Section

The reorganization and expansion provides additional resources and realigns CAB’s sections to better meet provider needs.

The CAB mailing address and mail stop code will remain the same; however, the email address has changed to CAB@cdph.ca.gov. For inquires related to licensure forms, application status, and facility renewals contact the CAB, Monday through Friday from 8:00 a.m. to 5:00 p.m. at (916) 552-8632 or by email.
