Stop and Read:

If this is for a Renewal, do not click Start New Application.  A new application will result in a new State ID and a gap in licensure which may affect your billing.

To Renew, refer to Search and Link Site (PDF) user manual on our Help page.

If you have already mailed-in an application, do not submit another application online as this will cause delays.


How do you apply?

Before starting an online application, review the checklist.

Step 1: Get prepared.

  • Download the New License Application Checklist (PDF) and refer to the appropriate certificate type.
  • See Fee Schedule (PDF) to determine the appropriate fee. Note: The federal CLIA program will bill you separately for the CLIA certification.

Step 2: Learn how to use the new online application system.
