​ How to Submit a Multiple Site License Application

 D. Select Sites to Include in the Application
             a. Add Site (Secondary Testing Site #1, #2, #3, etc.)

Click Add Site button to add a secondary site.

To add more sites, go back to Steps 15-28.

Click Continue Application button.

 E. Select Multisite License Qualification

After all sites have been added for this multiple site license application, click Yes.

Select multiple-site criterion. In this example, #2 is selected.

Select all sites that will be included in this application.

Select Yes for the Primary Site, and No for all Secondary Sites. Note:There should only be one Primary Site.

Click Next.

 F. Review Business Information (last chance to edit)

On this screen, you will have the chance to edit your Business Information. Scroll down to review. Click Edit if necessary. Click Next to confirm the information and proceed.

 G. Review Primary Testing Site Information (last chance to edit)

On this screen, you will have the chance to edit your Testing Site Information. Scroll down to review. Click Edit if necessary. Click Next to confirm the information and proceed.
