PREFIX	TYPE OF LABORATORY	DESCRIPTION
CDA	LIC	Deemed Status – Licensed Laboratory Physician Office Lab (POL) with federal Certificate of Accreditation (COA).
CDF	LIC	Deemed Status – Licensed Laboratory for a non-Physician Office Lab (POL) with federal Certificate of Accreditation (COA).
CLA	LIC	Licensed Laboratory Physician Office Lab (POL) with federal Certificate of Accreditation (COA).
CLF	LIC	State Licensure for a non- Physician Office Lab (POL) with federal Certificate of Accreditation (COA) or Certificate of Compliance (COC).
CLM	LIC	Licensed Laboratory for Physician Office Lab (POL) with federal Certificate of Compliance (COC).
